Management of Bone Infection, Nonunion, Malunion and Deformity Correction

Complex bone conditions such as bone infection, nonunion, malunion, and skeletal deformities can significantly affect mobility, limb function, and quality of life. These conditions may develop following severe fractures, previous surgery, trauma, infection, congenital abnormalities, or problems with bone healing and alignment.

Management of Bone Infection

Bone infection, also known as osteomyelitis, can occur following an open fracture, surgery, injury, or through other routes. Chronic infection may be associated with persistent wounds, drainage, bone destruction, or failure of a fracture to heal.

Management may require a combination of clinical evaluation, imaging, laboratory investigations, microbiological assessment, antibiotics, surgical removal of infected or non-viable tissue, and stabilization or reconstruction where appropriate.

In complex cases, treatment may need to be performed in stages, with regular monitoring to assess infection control and bone healing.

Nonunion and Delayed Bone Healing

A fracture that does not heal within the expected period may develop delayed union or nonunion. This can result in persistent pain, instability, deformity, or difficulty using the affected limb.

Treatment depends on the underlying cause and may involve improving mechanical stability, addressing infection when present, correcting alignment, bone grafting or other biological techniques, and specialized fixation or reconstruction procedures when required.

Management of Malunion

Malunion occurs when a fracture heals in an abnormal position, potentially resulting in angular deformity, rotational abnormality, limb shortening, altered joint mechanics, or functional limitations.

After detailed assessment, corrective treatment may be planned to improve bone alignment and restore more appropriate limb mechanics. Depending on the deformity, treatment may involve corrective osteotomy, gradual correction, external fixation, internal fixation, or other reconstructive techniques.

Deformity Correction

Bone and limb deformities may be congenital or develop following trauma, infection, previous surgery, or abnormal fracture healing. Deformities can involve angulation, rotation, translation, shortening, or a combination of these abnormalities.

Complex deformities may require detailed clinical and radiological planning. In selected cases, specialized techniques such as Ilizarov circular fixation or TSF software-assisted deformity correction may be considered for gradual and controlled correction.

Complex Limb Reconstruction

Some patients have a combination of problems, such as infected nonunion with bone loss, malunion with limb shortening, or deformity associated with previous surgery. These cases require comprehensive planning rather than treatment of a single problem in isolation.

Depending on the individual condition, treatment may involve staged reconstruction, bone defect management, deformity correction, limb lengthening, specialized fixation, soft-tissue management, and rehabilitation.

Rehabilitation and Long-Term Follow-Up

Successful treatment of complex bone conditions often requires regular clinical and radiological follow-up. Physiotherapy and rehabilitation may help maintain joint mobility, muscle strength, flexibility, and functional movement throughout the recovery process.
